Мужчине из Австралии были четкие симптомы инсульта. Его глаза подергивались вправо, при этом сам он шел неустойчиво и падал в правую сторону. Кроме того, кровоизлияние в мозг показала компьютерная томография. Поэтому медики сразу же поставили диагноз и назначили лечение. Можно сказать, что в какой-то степени ему повезло, так как инсульт является одним из тех болезней, которое часто имеет нечеткие симптомы. В результате врачи нередко ставят неправильный диагноз, который приводит к смерти пациентов.
В данном случае врачи не ошиблись, однако неясна была причина кровоизлияния. Ишемический инсульт обычно происходит в результате злоупотребления наркотическими средствами, такими как амфетамин, а также при раке мозга, физических травмах или чрезвычайно низких температурах. Однако у мужчины не было видимых причин для такого инсульта. Как сообщается в издании BMJ Case Reports, скорее всего, кровоизлияние было вызвано сильным повышением давления после стоматологической процедуры. Почему у пациента произошел инсульт после вырывания зубов?
Когда врачи начали изучать историю болезни, они обнаружили, что за полтора месяца до визита к стоматологу мужчина был направлен на компьютерную томографию, так как терапевт заподозрил у него нейродегенеративное заболевание, а именно — болезнь Паркинсона. Никаких проблем с сосудами головного мозга медики не увидели, но зато обнаружили признаки болезни белого вещества.
Заболевание CADASIL увеличивает риск инсульта
При этом заболевании, снижение кровотока приводит к повреждению нервных волокон мозга. Из-за этого информация начинает плохо передаваться между разными его частями. Диагноз мужчины впоследствии был подтвержден при помощи МРТ.
Чтобы выяснить причину заболевания, медики проанализировали ДНК пациента и нашли у него мутации в гене NOTCH3. Это наследственное заболевание, именуемое CADASIL, при котором утолщаются стенки сосудов, в результате чего ухудшается их проходимость и, соответственно, снижается приток крови к мозгу.

Ранее считалось, что кровоизлияние является редким осложнением CADASIL. Однако, согласно недавнему исследованию, осложнение может быть гораздо более распространенным. Но при этом медики утверждают, что это первый случай кровоизлияния у пациента с CADASIL после стоматологической процедуры. Причина, из-за которой вырывание зубов привело к сильному повышению давления, окончательно еще неизвестна. Поэтому ученым еще предстоит разобраться в произошедшем. Можно было бы допустить, что посещение стоматолога и инсульт были совпадением, однако удивительно, что эти два события последовали сразу друг за другом. Поэтому, скорее всего, взаимосвязь все же существует.

Но, в любом случае, переживать относительно этой опасности не стоит, так как CADASIL является редким заболеванием. В мире фиксируется всего 2-5 случая на 100 тысяч человек. Это значит, что у каждого из нас гораздо выше вероятность столкнуться с инсультом из-за недосыпа или любого другого фактора риска. Что касается мужчины, после инсульта пошел на поправку. Спустя три месяца он сообщил, что зрение восстановилось, а также прошли другие симптомы.
