Коронавирус мутирует в процессе лечения
В недавнем исследовании ученые проанализировали более 15 миллионов геномов SARS-CoV-2, и пришли к выводу, что препарат молнупиравир вызывает “мутационную сигнатуру”. То есть вызывает мутации в геноме вируса. Если вирус не полностью уничтожен в организме человека курсом лечения, то в мутированном виде может передаваться другим людям.
Мутации возникают по той причине, что препарат препятствуют репликации вируса. Проще говоря, ошибки возникают в момент создания новых копий вирусов. К слову, о мутагенных свойствах Молнупиравира было известно и ранее, однако теперь ученые выяснили, что препарат способен вызывать сильные мутации, но при этом SARS-CoV-2 сохраняет способность заражать людей.
Молнупировир, возможно, помогает вирусу эволюционировать
Как сообщается в исследовании, опубликованном в журнале Nature, мутационная сигнатура чаще всего встречалась в странах, где широко распространено использование данного препарата для лечения COVID-19. К таким странам относится США и Великобритания. В то же время в Канаде, где молнупиравир используется не часто, у вируса гораздо реже встречалась и мутационная сигнатура.
Почему опасны мутации коронавируса
Как и предполагалось, SARS-CoV-2 способен преподносить сюрпризы, и все это из-за его способности мутировать. Но опасность представляют не сами по себе мутации, а их способность помогать вирусу эволюционировать, то есть заражать и адаптироваться к иммунной системе. Дело в том, что не всегда мутации идут на пользу вирусу. И в нынешнем исследовании не так много доказательств того, что молнупиравир способствует эволюции SARS-CoV-2.

Кроме того, исходя из полученных данных, трудно количественно оценить, насколько часто линии SARS-CoV-2, возникшие в результате воздействия молнупиравира, распространяются между людьми. Если какой-то вируса с мутационной сигнатурой молнупиравира обнаруживается в базе данных, но у него нет близких родственников, ученым очень сложно определить его историю.
Некоторые эксперты обращают внимание на то, что в исследовании также не рассматривались потенциальные риски и преимущества использования препарата для отдельных пациентов. Давно известно, что у людей с ослабленной иммунной системой COVID-19 протекает дольше, что становится причиной мутаций самих по себе. Причем эти мутации могут быть более опасными, так как возникают естественным образом.
Тем не менее, в исследовании говорится, что некоторые из повторяющихся мутаций, скорее всего, все же могут помочь вирусу уклониться от иммунной системы. Это говорит о том, что препарат имеет потенциальную опасность. Но, чтобы получить более точные ответы на все вопросы, ученым необходимо провести дополнительные исследования.

Например, в будущих исследованиях авторы могли бы наблюдать за отдельными пациентами, которые принимают молнупиравир. Это позволило бы получить больше доказательств способности вируса эволюционировать. Если такая угроза действительно существует, должны быть приняты соответствующие меры, иначе вирус когда-нибудь действительно выйдет из-под контроля.
