Пересадка органов животных человеку называется ксенотрансплантацией. До последнего момента в мире был лишь один случай пересадки модифицированного свиного сердца. На этот раз свиное сердце было пересажено 58-летнему мужчине Лоуренсу Фосетту. Как сообщается, состояние мужчины было критическим, но из-за сосудистых заболеваний и риска внутреннего кровотечения его признали непригодным для традиционной трансплантации. Поэтому ксенотрансплантация стала для него единственным шансом на жизнь.

Американские ученые успешно пересадили свиное сердце второму пациенту
Пересадка свиного сердца человеку
Первым пациентом, которому пересадили сердце свиньи стал Дэвид Беннетт. Операция была осуществлена в начале 2022 года. Для этих целей ученые вырастили специальных генно-модифицированных свиней, которых модифицировали по технологии CRISPR, благодаря чему их органы не вызывают отторжения. Но, как мы уже сказали, мужчина прожил со свиным сердцем не долго. Как выяснилось впоследствии, основной причиной его смерти стал свиной вирус.
Учтя предыдущие ошибки, ученые подготовились ко второй операции и нашли пациента, которым стал Лоуренс Фосетт. Он был госпитализирован 14 сентября из-за осложнения, связанных с сердечной недостаточностью и заболеванием периферических сосудов.
Прежде чем дать согласие на трансплантацию, он был проинформирован о рисках, а также о том, что процедура является экспериментальной, поэтому все риски до конца неизвестны. Но, как мы сказали выше, у мужчины, фактически, не было выбора. Перед операцией ему пришлось пройти психиатрическую экспертизу и встретиться со специалистом по медицинской этике, социальными работниками и другими членами медицинской комиссии, давшими в итоге разрешение на пересадку органа.

Модифицированную свинью предоставила компания United Therapeutics Corporation. Операцию и первому и второму пациенту проводил доктор Бартли Гриффит, заслуженным профессор трансплантационной хирургии Томаса, клинический директор программы сердечной ксенотрансплантации. Как сообщается, пациент после трансплантации дышал самостоятельно. Сердце функционировало хорошо без помощи каких-либо вспомогательных устройств.
Чтобы не допустить отторжения нового сердца, мужчина принимал специальные препараты, а также прошел курс терапии антителами. Как сообщают сами ученые, речь идет о совершенно новой терапии, разработанной биофармацевтической компанией Eledon Pharmaceuticals. Она основана на экспериментальном антителе под названием тегопрубарт. Это антитело блокирует белок CD154, который участвует в активации иммунной системы.

Если второй эксперимент по пересадке сердца свиньи человеку пройдет успешно, то есть мужчина сможет жить со свиным сердцем так же как с человеческим, это станет серьезным прорывом в медицине, который позволит решить проблему донорских органов. В настоящее время только в США в очереди на органы стоят сотни тысяч человек. Более 6000 пациентов ежегодно умирают так и не дождавшись своей очереди.
Поэтому трансплантация органов животных потенциально может спасти тысячи жизней, но в то же время она пока несет серьезные риски. Помимо страха передачи неизвестного патогена от животного к человеку, ксенотрансплантаты с большей вероятностью могут вызвать опасный иммунный ответ. Эти реакции может вызвать немедленное отторжение органа с потенциально смертельным исходом для пациента.
