Обычно медикаментозное лечение депрессии длится не менее шести месяцев после того, как больной начинает чувствовать себя лучше. В случаях, когда депрессия плохо поддается терапии или же является хронической или тяжелой, лечащий врач может порекомендовать длительный прием антидепрессантов. И все же, почему лечение антидепрессантами длится так долго, а эффект наступает не сразу?

Почему антидепрессанты работают не сразу?
Результаты нового исследования, презентованного в ходе конференции European College of Neuropsychopharmacology показали, что замедленная реакция антилепрессантов на лечение имеет биологические причины. Отметим, что на протяжении долгого времени было неясно, почему препаратам из группы СИОЗС требуется так много времени – обычно несколько недель – для достижения заметных результатов.
В ходе работы команда измерила физические изменения в нейронных связях (синапсах) после лечения СИОЗС у здоровых взрослых. Исследователи обнаружили, что «у тех, кто принимал СИОЗС, со временем наблюдалось постепенное увеличение количества синапсов в неокортексе и гиппокампе головного мозга», – объясняет нейробиолог Гитте Кнудсен из Копенгагенской университетской больницы.

Напомним, что повышение уровня нейромедиатора серотонина в мозге отвечает за эффект СИОЗС, повышающий настроение. К сожалению, они действуют не на всех, и ученые пока не знают точного механизма их действия. Ранее была выдвинута гипотеза о том, что СИОЗС повышают синаптическую пластичность в мозге. Считается, что эта способность синапсов усиливаться или ослабевать с течением времени важна для обучения, памяти и регуляции настроения.
Антидепрессанты и нейропластичность
Чтобы проверить, верна ли гипотеза о нейропластичности, исследователи провели двойное слепое полурандомизированное контролируемое исследование с участием 32 взрослых без депрессии в анамнезе. Участникам случайным образом назначили ежедневный прием препаратов из группы СИОЗС по 20 миллиграммов и плацебо в течение 5 недель.
По истечении указанного времени, участникам исследования провели позитронно-эмиссионную томографию (ПЭТ) для измерения уровня в мозге белка 2A (SV2A), уровень которого указывает на наличие синапсов. Более высокие уровни в определенной области означают большую плотность синапсов в этой области.
Сканирование выявило, что у людей, принимавших антидепрессант, были более высокие уровни белка SV2A в неокортексе и гиппокампе по сравнению с теми, кто принимал плацебо, – говорится в работе.

«Задержка терапевтического действия антидепрессантов была загадкой для психиатров с тех пор, как они впервые были обнаружены более 50 лет назад», – говорит Дэвид Натт, нейропсихофармаколог из Имперского колледжа в Лондоне, который не принимал участия в исследовании. Таким образом, новые данные окончательно доказали эффективность антидепрессантов в лечении депрессии и объяснил причины, по которым эффект от лечения наступает не сразу.
