Ксенотрансплантацию, то есть пересадку человеку органов свиньи, ученые считают перспективным направлением трансплантологии, которая сделает ее более доступной для нуждающихся людей. Однако до последнего момента удачных операций было не так уж много. Например, ученые два раза пересаживали свиное сердце, но спустя некоторое время оба пациента умирали. Попытки пересадить почку уже совершались несколько лет назад, но операции проводились на людях, у которых уже умер мозг, то есть исключительно в тестовом режиме. Лишь весной 2024 года впервые почка была пересажена живому человеку, и он остался жив, однако об окончательных результатах пока говорить рано, так как прошло слишком мало времени. Тем не менее медики решили пошли еще дальше — они пересадили почку и вилочковую железу от свиньи человеку, а также имплантировали сердечный насос. Такая операция была проведена впервые в истории медицины.

Почему некоторым людям необходима пересадка свиных органов
Обычно ученые прибегают к пересадке органов свиньи человеку в крайних случаях, когда других шансов спасти жизнь не существует. Так было при пересадке сердца 58-летнему мужчине Лоуренсу Фосетту. Так же произошло и на этот раз — пациенткой стала 54-летня Лиза Пизано, у которой была диагностирована сердечная недостаточность и последняя стадия заболевания почек, требующая регулярного диализа для очистки крови.
Однако женщина не имела право на пересадку человеческих органов, потому что у нее было несколько хронических заболеваний. Как сообщают ученые, она была “плохим кандидатом”. Дело в том, что количество человеческих органов ограничено, в результате чего многие люди длительное время ждут своей очереди. Поэтому, чтобы претендовать на пересадку органов, люди должны соответствовать определенным требованиям, которые предполагают, что донорские органы не будут использованы зря.
Кроме того, из-за особенностей организма пациентки, найти почку, которая бы соответствовала ее иммунной системе, было очень сложно. Как сообщают ученые, ей пришлось бы ждать донорский орган годами.

Пересадка сердечного насоса и свиной почки
Как уже было сказано, попытки пересадить свиное сердце человеку закончились смертью пациента. Поэтому врачи из академического медицинского центра NYU Langone Health не рискнули совершить такую пересадку, а приняли решение использовать механический сердечный насос, то есть вспомогательное устройство для левого желудочка (LVAD). Однако выполнять эту операцию было рискованно, пока женщина “привязана” к диализу.
Как уже было сказано выше, в ближайшие несколько лет найти подходящую человеческую почку не было, но операция на сердце требовалась безотлагательно. Женщине оставалось жить всего несколько недель. В результате было принято решение пересадить свиную почку. Как мы ранее уже рассказывали, тестовые операции по пересадке почек прошли успешно. Кроме того, уже есть один пациент, который живет со свиной почкой.

В начале апреля женщина пережила две операции. Ей был имплантирован сердечный насос, свиная почка и вилочковая железа. По словам ученых, последняя помогает “тренировать иммунные клетки” организма. Проще говоря, вилочковая железа должна помочь предотвратить отторжение почки иммунной системой.

Как сообщают сотрудники NYU Langone Health, операция была уникальной по нескольким причинам. Во-первых, еще ни одному пациенту с LVAD никогда не пересаживали даже человеческую почку.
